Professional referrals

A clear first step for planned support.

A focused referral route for social workers, discharge teams, case managers, commissioners and other authorised professionals.

Referral service and CQC status

Personal Care is subject to CQC approval. We are applying for registration and will provide regulated Personal Care only after approval has been granted.

Start with the essentials

Share only what we need to review the referral.

Tell us who is referring, the planned area and timing, the funding route, the referral basis and a short summary of essential support needs.

Please do not send detailed clinical records, full addresses or dates of birth through this initial form. If the referral progresses, we will agree an appropriate route for further information.

Who can use this route?

  • Social workers and brokerage teams
  • Hospital discharge and community health teams
  • Case managers and care coordinators
  • Commissioners and funding professionals
  • Other authorised professional referrers

Individuals, relatives and friends should use the standard contact page.

Keep this concise. We will agree a secure route for supporting records if the referral progresses.
