A strengths-based approach

Doing with, not simply doing for.

Support should preserve skills, confidence and choice. Where safe and wanted, we encourage people to take an active role in cooking, dressing, moving around, planning their day and staying connected.

The care plan records how to offer the right amount of help without unnecessarily taking control away.

From enquiry to review

How a future care package would be arranged.

01

Initial conversation

We listen, answer questions and identify whether a fuller assessment would be useful.

02

Needs assessment

We explore health, wellbeing, routines, strengths, risks, preferences and desired outcomes.

03

Clear proposal

You receive an explanation of what can be provided, when, by whom and at what agreed cost.

04

Ongoing review

Care is reviewed with you and adapted when needs, outcomes or circumstances change.

Who we aim to support

Adults who want to live well in their own home.

Adults

Older adults

Support with daily routines, confidence, connection and independence at home.

Recovery

People returning home

Time-limited help after hospital discharge or a change in circumstances.

Flexible

People with changing needs

Responsive support where an assessment confirms our team can meet needs safely.

Together

Families and unpaid carers

Planned respite, reassurance and communication in line with the person’s wishes.

Acceptance of any care package depends on our registration status, an individual assessment, compatibility with our service scope and safe staffing capacity.
